Care Navigators

Across the Camden PCN we have Care Navigators available to support our patients. They can help you to find the right care and support. They guide people to the right services, coordinate between professionals, and give short-term help with complex needs. This can include referring and linking vulnerable adults to services such as district nurses, falls teams, mental health support or to adult social care for care act assessments. Their role makes care easier to access and ensures patients feel supported. 

If you think this is a service that you could benefit from or need, you can either ask your GP to refer you to one or you can self refer via Age UK Camden | Care Navigation and Social Prescribing

 

Social Prescriber

Also working across the PCN we have Social Prescribers who help with non-medical issues that affect health, like loneliness, money worries, or lack of support. They link people to local groups, charities, and services, so patients get the right help without always needing a GP. This can include signposting or referrals to things like social groups, activities, counselling, befriending, volunteering, work support, advice services, and carer support.
