Pre-registration with the practice
If you wish to register please come along during the following times:-
Mondays, Wednesdays and Thursdays: 8am - 5pm
(Registrations may take longer on Monday morning as this can be a particularly busy day).
Tuesdays: 8am - 7:15pm
Fridays: 8am - 4:00pm
Please note: The Practice & Telephone line is closed from 1pm-2pm everyday for staff meetings and clinical administration.
Please bring with you,
- Your completed form including NHS number (you can get your NHS number from your previous practice)
- Two current proofs of address no older than 3 months (driving licences are not accepted as a proof of address)
- Passport (for identification purposes)
The registration process will take about 15 minutes per person as long as all the information is correct. When you register you will also be asked to provide a urine sample (sample bottles are available at the reception desk, you can help yourself).
Your height, weight and blood pressure will also be recorded.
Please print off the GMS1 form and adult or child registration form. Complete these forms, sign and date, and hand to a receptionist upon arrival.
